HART (Homeless Alliance Response Team)

The HART Outreach Service is a Primary Care Health Service provided by Rochdale Health Alliance (RHA) for the homeless and transient community within our borough.

The HART Outreach service consisting of two nurses and a Liaison Officer will continue providing clinical interventions and assessment.

Our sessions are listed below and will be updated as the service develops. We are also ensuring our clinical sessions fully comply with Covid-19 regulations.

• Partnership working with other agencies
• Weekly hostel visits
