Living Well with Long-Term Conditions Programme

People living with long-term conditions, disabilities and re-occuring illness are challenged daily by symptoms of their condition.

Taking control of our own health and wellbeing, as well as knowing when to ask for help is essential if we want to maintain fitness and learn to live well. The courses we offer help to support and enable you to do this. 

Access to the course is free providing you are 18+ and registered with a GP in Heywood, Middleton, Rochdale or Littleborough.
